Beth Din of East Flatbush

Thank you for choosing the Beth Din of East Flatbush. This form is designed to provide our office with the information needed to better understand your matter and prepare for the mediation process.

Mediation provides the parties with an opportunity to discuss their concerns in a structured and respectful setting with the assistance of a neutral mediator. Our goal is to help the parties communicate effectively, identify the issues that need to be addressed, and work toward a mutually acceptable resolution whenever possible.

Before You Begin

Please carefully complete all sections that apply to your situation. Provide accurate and complete contact information for each party and include any additional details that may help us understand the nature of the matter.

During this process, you may also be asked to select the type of mediation or service requested, review applicable fees and responsibilities, and acknowledge the terms of the mediation agreement.

Confidentiality & Good Faith Information provided through this form will be handled with discretion. The parties are expected to participate honestly and in good faith and to provide complete and accurate information throughout the mediation process.

Important Information

Mediation is intended to assist the parties in reaching an agreement. The mediator serves in a neutral and facilitative role and does not represent either party as personal legal counsel unless a specific service agreement expressly states otherwise.

Please review all terms, fees, payment responsibilities, and acknowledgments carefully before submitting or signing an agreement. If an agreement is ultimately reached, additional documentation and signatures may be required.
